2. ONLY IF your lost/stolen passport is valid:

- Completed Department of State Passport Form – 64. (File size: 115KB)

- Fill in as much of the passport information as you can.

- Answer all the other questions in detail.

3. Original proof of US citizenship.

- Certified birth certificate or naturalization certificate or passport issued over 15 years ago.

4. 2 recent passport size (2x2) photographs.

5. Authorization letter for rush services.

6. Copy of itinerary for rush services.

7. Processing fees: $175.00 for rush – 1-2 working days.

8. Service charges: $125.00 for rush.

9. Return Postage: $22.00 Priority overnight (up to 3 passports)

FedEx Saturday Delivery: $30.00
